• City - Enter the city of the employee's address.
• State - Enter the state of the employee's address.
• Postal Code - Enter the zip code of the employee's address.
• Country - Enter the country of the employee's address.
• Home Phone - Enter the employee's phone number

Emergency Contact

• First Contact - Enter a contact name in the event of an emergency.
• Day Phone - Enter the phone number of the contact.
• Relationship - Select what relationship the contact is to the employee.
• Second Contact - Enter a contact name in the event of an emergency.
• Day Phone - Enter the phone number of the contact.
• Relationship - Enter what relationship the contact is to the employee.

Additional Information

• Date of Birth - Enter the employee's birthday.
